Entry inside a rated zone, confirmation nearby, within your trade limit.

Positive evidence of a break — open-air entry, no confirmation, or past the cap.

No data

Monitoring wasn't running, so there's no record either way. Never counted against you.

Absent evidence is not a rule break. If a missing record counted as a failure, the journal would call you undisciplined on days you may have traded perfectly — and you'd "fix" behaviour that was never wrong. A journal that invents faults is worse than no journal.
